Notice of Availability for IRS Form 1095-C

This notice is to inform you that Franklin County will not automatically mail an IRS Form 1095-C, *Employer-Provided Health Insurance Offer and Coverage*, for the 2025 tax year to all employees and covered individuals.

This change is allowed under a recent federal law, the Paperwork Burden Reduction Act.

Your Right to a Form 1095-C

You have the right to request a copy of your Form 1095-C for the 2025 tax year. This form provides information about the health coverage offered to you and your enrollment status during the calendar year.

How to Request Your Form

If you would like a paper or electronic copy of your Form 1095-C, you must submit a request using one of the contact methods below:

Your request must include your full name and mailing address (if you are requesting a paper copy).

Response Time

Upon receiving a request, Franklin County will furnish a copy of the requested Form 1095-C no later than 30 days after the date your request is received.

Important Dates

This notice will remain posted on the Franklin County website through at least October 15, 2026. Franklin County remains obligated to file all required information with the Internal Revenue Service (IRS).
